Take the Shahada
The Shahada is the declaration of faith and the first of the Five Pillars of Islam. Reciting it sincerely before witnesses is all that is required to enter the faith.
أَشْهَدُ أَنْ لَا إِلَٰهَ إِلَّا ٱللَّٰهُ وَأَشْهَدُ أَنَّ مُحَمَّدًا رَسُولُ ٱللَّٰهِ
Ashhadu alla ilaha illa Allah, wa ashhadu anna Muhammadan rasulullah
"I bear witness that there is no god but Allah, and I bear witness that Muhammad is His messenger."
