I intend to participate in certain events, activities, presentations, seminars, panel discussions and the like (each, an “Event” and collectively, the “Events”) organized, sponsored, or promoted by the Greater San Diego Muslim Community Center (“MCC”), a nonprofit organization located at 14698 Via Fiesta, San Diego, CA 92127.
For each such Event, I hereby grant MCC the right to record my voice, likeness, image, presentation, and presented information in any format (e.g., video, audio, photography) (collectively, the “Recordings”); to make derivative works thereof (“Derivative Works”); and to show or share the Recordings or Derivative Works (e.g., on MCC’s website and social media channels). I acknowledge and agree that MCC will have exclusive ownership of all Recordings and Derivative Works, and I waive all claims to compensation for the use of the same. I release and discharge MCC and its representatives from any claims, liabilities, or damages arising out of the use or distribution of the Recordings and Derivative Works as outlined in this agreement. MCC agrees to make best efforts to give me attribution credit for the Recordings and Derivative Works.
This agreement does NOT give MCC ownership over materials that I created prior to the Event or present during the Event. I represent and warrant that all materials I provide or present are my original work or properly credited to their respective owners, with necessary permissions secured. I will not disclose any confidential or proprietary information during my presentation unless specifically authorized by the owner.
Opt-Out Clause. To the extent that I wish to exclude any specific material, remarks, or likeness from this agreement (“Excluded Materials”), I shall provide written notice (“Opt-Out Request”) to the board of directors of MCC at least 24 hours prior to the Event in which such Excluded Materials will be used or presented. Opt-Out Requests shall be by email sent to BOD@mccsandiego.org and shall identify with specificity the items to be considered Excluded Materials for an Event. If an Opt-Out Request is not timely received, MCC may in its discretion honor such notice on a case-by-case basis, but in no event will MCC’s exercise of discretion be considered a waiver of this paragraph or of MCC’s rights with respect to any other Event or any other Content or materials. An Opt-Out Request must be received with respect to each Event for which Speaker wishes Excluded Materials to be excluded from this agreement.
Acknowledgment. I have read this Speaker Consent and Waiver Form in its entirety. By signing below, I agree to the terms outlined above and acknowledge my understanding and consent. I may cancel this agreement at any time by sending an email to BOD@mccsandiego.org, but the rights granted herein with respect to Events before any such cancellation shall survive such cancellation.
